Reading a thread at my favorite programmer's forum http://rsdn.ru/forum/job/4664279.1.aspx, can't help but to recall mine best.
I was writing an e-mail to a technical lead saying "I have found a critical issue in a code we just shipped to customer. It would take me about a week to fix it. We have two choices: to inform customer now, or to wait for a week and provide a data about bug and a fix altogether. If we choose the later option there is a risk that customer will encounter a bug too".
Then I sent this e-mail to this customer thanks to Outlook's auto completion (two names were almost the same).
